Therapy Dog Program Request Form

If you are interested in setting up a therapy dog program at your venue, please fill out the request form below to submit your inquiry for review. This includes our Paws to Read, Paws to Heal, and Paws to Comfort programs. The request must come from a representative of the organization or venue requesting the program.

Please be aware that we currently have a waitlist for new program requests. Completion of the form does not ensure that we will be able to partner with you for a new program or attend your event. After you submit your request, a Paws & Think Program Coordinator will contact you to discuss further.

This form is for reoccurring visit requests. If you are looking to request a one-time/special event request, including events with and without dogs, you can fill out this form.
